Personal Supports

CHI provides a highly individualized array of supports customized to meet the diverse needs of men and women living in their own home with their families, living alone or with housemates.

Each person who is supported at CHI participates in an annual Individual Planning meeting with invited family, friends and professionals. At this meeting, an Individual Plan (IP) is developed which outlines and articulates the desired and needed supports and services, the number of hours of funded support and when (times and days of the week) the services and supports will be delivered. Additions, modifications and changes can be made as people’s needs change and/or they reach or exceed their established personal goals. We know that our work is not done until you are satisfied.

The plan is truly tailor-made to perfectly fit the ever-changing desires, wishes and needs of each person supported.

Personal Supports may include:

    • Individualized support and assistance with personal care (including personal hygiene & grooming, clothing selection).

    • Individualized support and assistance with household management (including laundry, cleaning, meal planning, shopping & cooking, etc.).

    • Individualized support and assistance with making and keeping appointments (including medical appointments, Social Services, etc.).

    • Individualized support and assistance to attend classes.

    • Individualized support and assistance to develop new skills (including parenting skills, problem solving strategies, etc.).

    • Individualized support and assistance to develop, nurture and grow healthy relationships (family, friends and others).

    • Individualized support and assistance to access community resources.
